If you are having difficulty placing your guests’ names and company names on the guest list form linked above, as an option, you can email us a word document with the information or just write directly (must be neatly) on the form and email it to events@mwmca.org or fax it to 1-888-609-8318. Providing us with your guests’ names and company names allows us to provide your guests with accurate name badges. Each guests’ name badge will serve as their ticket, and will be picked up at event registration.
